What causes sensitive teeth ?

As your local dentist we’re here to help you with your dental concerns. Teeth may be sensitive for a number of different reasons. Teeth are often sensitive when dentine is exposed.

Dentine is the layer of tooth structure under the outer enamel layer. It has channels(tubules) that go to the inner nerve part of the tooth.

Dentine is often exposed near the gum line, when the gums pull away from the teeth or when gum tissue is lost. Dentine can also become exposed if the outer enamel layer on teeth wears away.

Below are some factors that can contribute to sensitive teeth :

  • Tooth decay : This is a common cause of pain on hot, cold or sweet. Treatment may require a filling to be placed in the tooth.
  • Brushing too hard or using a toothbrush with hard bristles will lead to gum recession and/or tooth wear.
  • Sugary and acidic foods and drinks : this can lead to erosion tooth wear by wearing away the enamel.
  • Teeth grinding : this can lead to the tooth surface wearing away.
  • Gum disease : this can lead to gum recession. ( the gums shrink back )
  • A cracked tooth or filling : A crack can run from the outer surface of the tooth towards the root and the inner part of the tooth.

Caring for your baby’s teeth …

Here’s some tips to help you look after your baby’s teeth up to the age of 3 years old.

When to bring your baby to the dentist

You should bring your child to the dentist from a young age. That way they get used to the environment and are relaxed when attending future visits. This was highlighted by the Royal College of Surgeons, who recommended ‘ Parents must make sure their children visit a dentist by the age of one. ‘ http://www.standard.co.uk/news/health. It was also recently revealed that tooth extractions in children 4 years age and under rose by 24 % in the last decade  https://www.theguardian.com . A further study showed, 80% of under-twos in England failed to visit the dentist.

After their initial visit, they should attend regularly, as often as recommended by the dentist.

Baby teeth usually start breaking through the gums at around 6 months age. The teething process may continue on until about 2-3 years by which time they will usually have 20 baby teeth. Growing teeth can often cause discomfort. Chilled teething rings may help soothe the sore gums. DO NOT place anything sweet on the teething ring.

Dummies should also be avoided as they can become a habit that can affect the alignment of teeth.

How to clean your baby’s teeth

Parents/carers should brush or supervise toothbrushing.

As soon as teeth grow into the mouth, brush them twice daily with a fluoridated toothpaste. If it’s difficult to use a toothbrush, consider using a guaze or flannel to at least wipe teeth rather than not cleaning them at all.

Brush last thing at night and on one other occasion.

Use a soft toothbrush designed for young children and brush for about 2 minutes gently in small circular motions.

Use fluoridated toothpaste containing no less than 1,000ppm fluoride, that is age related specific.

It’s good practice to use only a smear of toothpaste as it’s unlikely your child will be able to spit out properly.

Avoiding tooth decay

Breast feeding provides the best nutrition for babies.

Introduce infants once at the age of six months to drinking from a free-flow cup. Once your child is one years old try to discourage feeding from a bottle.

Sugar should not be added to weaning foods or drinks.

Reduce the frequency and amount of sugary food and drinks. Try to get your little one into good habits from a young age. Milk or water between meals are better options.

If your child needs to take medication, try to give them sugar-free medicines.

What is Cracked Tooth Syndrome ?

Unlike teeth with obvious fractures, teeth with cracked tooth syndrome usually have fractures that are too small to be seen on X-rays. Sometimes the fracture is below the gum line, making it even more difficult to identify.
Cracked tooth syndrome more often occurs in molars. Eating hard foods such as nuts, may make you more susceptible to cracking a tooth.
People who grind or clench their teeth may also be more susceptible to cracked tooth syndrome because of the constant forces put on their teeth. Sometimes a person’s normal bite causes certain molar cusps (the highest points of the tooth) to exert so much pressure on the opposing tooth that it cracks. Actress Demi Moore revealed in a Tonight Show interview, that she lost front teeth due to stress and grinding her teeth. Actress Charlize Theron also cracked 2 teeth due to clenching her jaw so hard while filming.
Teeth with large fillings or teeth that have undergone root canal treatment are weaker than other teeth and may be more likely to crack. People with one cracked tooth are more likely to have others, either at the same time or in the future.

Symptoms

You may experience pain in the tooth when you bite or chew. However, it probably will not happen all the time. The tooth may be painful only when you eat certain foods or when you bite in a specific way. You will not feel a constant ache, as you would if you had a cavity or abscess, but the tooth may be more sensitive to cold temperatures. If the crack worsens, the tooth may become loose.
Many people with cracked tooth syndrome have symptoms for months, but it’s often difficult to diagnose because the symptoms are not consistent.

Diagnosis

Diagnosis of cracked tooth syndrome is often difficult. X rays often do not show the crack.

Expected duration

How long symptoms last depends somewhat on how quickly a cracked tooth can be diagnosed. Even then, treatment may not always completely relieve the symptoms.

Prevention

Grinding or clenching your teeth can increase your risk of cracked tooth syndrome. A mouth guard may be advisable.
Hard foods such nuts may cause a tooth to crack. Avoiding placing objects in the mouth such as trying to open bottles with your teeth.

Treatment

Treatments for cracked tooth syndrome do not always completely relieve the symptoms.
Treatment depends on the location, direction and extent of the crack. Cracks vary from superficial ones in the outer layers of the tooth to deep splits in the root affecting the pulp (the centre of the tooth, which contains the tooth’s nerves). Treatment can vary and options may include filling, crown, root canal treatment or sometimes the tooth needs extraction. A consultation and examination is recommended in order to assess each individual situation so appropriate treatment options can be discussed.

Tooth wear … do you have it ?

A large portion of the population have some degree of tooth wear yet they are unaware of it.
How do I know if I have tooth wear ?

Often there are obvious signs of tooth wear, such as chipped teeth and changes in the tooth shape, or teeth appear yellower or darker in colour. There may be sensitivity to hot,cold or sweet. However sometimes the signs are not so obvious. These can be translucency or thinning of the edge of the front teeth, small dips in the biting surface of back teeth, or notches on the teeth near the gum line. Any of these signs indicate the protective outer enamel layer of the teeth has been damaged. Once the enamel has worn away it does not grow back.

What causes tooth wear ?

There are 3 main types of tooth wear – Erosion, Abrasion and Attrition. The outer hard enamel layer is worn away, and can lead to exposure of the inner softer dentine layer.

Erosion, sometimes known as Acid Wear, is commonly caused by acidic foods or drinks we consume in our diets, that dissolve away the tooth structure. A snacking culture along with demand of fresh fruit and juices/smoothies may have contributed to the increase in erosive tooth wear in recent times.
In certain patients, acid reflux may also be a cause.

Abrasion is caused by an object rubbed against the tooth surface, such as a toothbrush when brushing too hard, or an abrasive toothpaste.

Attrition occurs where teeth rub against each other such as a habit of grinding or clenching teeth.

How can I protect my teeth from the effects of tooth wear ?

During your dental check up with us, we check for tooth wear and advise accordingly. Treatments may vary from a prevention approach, to applying white fillings to protect damaged teeth from further damage. Once the outer enamel layer has worn away the inner dentine layer wears away at a quicker rate because it’s softer than the enamel. White fillings are a comfortable and conservative treatment option which allows placement of a protective covering whilst avoiding drilling the tooth. The white filling bonds to the underlying tooth structure. With white fillings starting from approximately £100 per tooth, they are a cost effective way to protect teeth from further damage and have the added benefit of improving your smile.

  3. Avoid brushing your teeth after having acidic food or drink as the enamel is softer at this time.
  4. Brush your teeth using a soft/medium bristled toothbrush twice a day. Do not put too much pressure when brushing and avoid using an abrasive toothpaste.
  5. Use a daily fluoride alcohol-free mouthwash.
  6. Have regular dental checkups to keep your teeth healthy and looking good.
